Login and Registration FAQs

If you have not logged in recently, you may be prompted to customize your account with a unique user name and password and to answer security questions to protect your account.

I don’t know if I’ve registered on this site already. How should I proceed?
If you’re unsure whether you are a registered user on My Program, please log in as a new user. We will recognize your information and direct you to the appropriate screen.

Where can I find my certificate number?
Your certificate number, alternately called your member number or customer ID number, can be found on various forms. When logging in, please enter only the numeric digits. Please scroll over file name to view location of certificate number in your files:
- invoice,
- certificate of insurance,

My e-mail has changed since I've registered. How do I login?

Please login using the e-mail address provided during the original registration process. You can change your e-mail address after you've logged in.

I have registered but can not remember my user name.

Please click here, and we will email your user name to the account your provided.

I have registered but can not remember my password.

Please click here to enter your user name, and we will email your password to you.

I can't remember my user name or password. What should I do?

With basic account information, you can Reset Your Password.

I cannot remember which email address I used when I first registered on the previous site.

Please contact us through our login client service form, and we will contact you with that information.

I am still having trouble logging into My Program. What do I do?

If your question is not addressed, please contact us via our login client service form. If you are unable to use this form, please contact us at FPOLogin@prms.com. In your message please give us the following information to help expedite the handling of your request:

  • The specific error message you are receiving
  • Your full name, city and state
  • The e-mail you are using to login
  • Your Customer ID number you are using to login
